In the premiere, tension mounted when the protagonist, neurosurgeon Kang Tae-ju (played by Namgoong Min), announced his intention to divorce his wife, Ko Se-yoon (played by Lee Seol), only to have her kidnapped immediately afterward.
In the drama, Kang Tae-ju was locked in a fierce conflict with his wife, Go Se-yoon—who is also the hospital’s chairwoman—and his father-in-law, the hospital’s founder, over the surgery of a VIP patient. After the conflict escalated, Kang Tae-ju demanded a divorce but was rejected. Later, while drunk and on his way home with a designated driver, Kang Tae-ju fell asleep in the car. The next morning, he received a threatening call from a mysterious man stating that his wife had been kidnapped and demanding a ransom of 1 billion won.
